Nicole Phelps, LCSW is only in-network with Moda Insurance and Cigna Insurance, otherwise services must be paid out-of-pocket by the client. Some insurance companies cover out-of-network costs, please contact your insurance provider directly to inquire. Client is responsible for submitting any out-of-network claims. Please don't hesitate to ask any questions regarding payment for services.
